It turns out that this is cause because Google does not populate the favorite icon when navigating image search results (see attached screenshot). Privacy Browser displays the last favorite icon shown, so it looks like it has one. But when navigating back it realizes it doesn't.

I am going to close this as it feels like a bug with Google's favorite icon code and not with Privacy Browser.
